For your patients with Medicare Part D
Teva’s Shared Solutions® is committed to helping your patients ﬁnd aﬀordable access to AJOVY. Patients with Medicare Part D may be eligible for the Patient Assistance Program (PAP) through Shared Solutions®, provided they have fulﬁlled the insurance requirements (eg, Prior Authorization, formulary exception request) and meet the income eligibility limits. How does my patient qualify for the Patient Assistance Program?
To qualify, the patient must:
- Apply for assistance
- Be a legal US resident
- Meet the insurance eligibility criteria
- Meet the income eligibility limits

How does my patient apply for the Patient Assistance Program?
Please direct your patient to the Patient Services Portal. From there, the patient can access a variety of services based on their needs. They can receive authorization for services, apply for financial assistance, enroll in copay assistance programs, and more.
